This is the MAIN form of the whole application — Notification form to Immigration (届出書), issued by the Immigration Services Agency. Every other document is an attachment to it; without it you cannot file at all.
What it must show
- Use the LATEST official version — old versions may be rejected
- Fill it in Japanese, clearly, with no corrections
- Attach the photo and sign in the designated boxes
- Some procedures have a section for your COMPANY / sponsor to fill in — ask them to complete it
Where to get it
Download the latest version from the Immigration Services Agency website, or pick it up at an Immigration counter. IIKANZI VISA can also fill it in for you and export a print-ready PDF.
How to get it
- Option 1: download the blank form from the Agency site and fill it in by hand
- Option 2: use IIKANZI VISA — answer step by step, the app fills the right boxes and exports a PDF
- Print it, sign, attach the photo, and submit together with the other documents
Cost & time
The form itself is free. A fee (paid with a 収入印紙 revenue stamp) applies only when your application is APPROVED — check the Agency's current fee notice.

All you do at this station is enter the right dates and your company details — the app fills in the 届出 form for you, ready to download at the final stop. The only choice left is whether to file online or by post.
What it must show
- Your residence card (card number + the address on the back).
- The leaving date on your resignation certificate, and the start date on the new contract.
- Name · address · phone number of both the old and the new company.
- The company's 法人番号 (13-digit corporate number) — ask HR if you don't know it.
Where to get it
① Online — fastest, no stamp needed. Go to Immigration's [電子届出システム](https://www.moj.go.jp/isa/applications/online/i-ens_index.html) → register an account once → log in → choose 「契約機関に関する届出」 → fill it in and send → screenshot the confirmation screen. ② By post. Print the form the app filled in for you (final stop), attach a copy of both sides of your residence card, and post it to the Tokyo Immigration Bureau — the next stations show you how to address the envelope and what to do at the post office. Where to get it
Three ways: ① Immigration's electronic notification system (online, fastest) · ② By post to the notification desk of the Tokyo Immigration Bureau — it handles the WHOLE country; write 「届出書在中」 on the envelope · ③ In person at your local Immigration Bureau.
Notes
⚠️ This notification does NOT replace a permission application. If the new job differs in content → you still need a status change. Tokutei 1 workers changing employer → the status change must be APPROVED before you start work (see the 🔀 Change employer procedure).
