What this document is
All you do at this station is enter the right dates and your company details — the app fills in the 届出 form for you, ready to download at the final stop. The only choice left is whether to file online or by post.
What it must show
- Your residence card (card number + the address on the back).
- The leaving date on your resignation certificate, and the start date on the new contract.
- Name · address · phone number of both the old and the new company.
- The company's 法人番号 (13-digit corporate number) — ask HR if you don't know it.
Where to get it
① Online — fastest, no stamp needed. Go to Immigration's [電子届出システム](https://www.moj.go.jp/isa/applications/online/i-ens_index.html) → register an account once → log in → choose 「契約機関に関する届出」 → fill it in and send → screenshot the confirmation screen. ② By post. Print the form the app filled in for you (final stop), attach a copy of both sides of your residence card, and post it to the Tokyo Immigration Bureau — the next stations show you how to address the envelope and what to do at the post office. Notes
A company name change / address change uses form 参考様式1の6 — the app does not carry that form yet, so download it from Immigration's procedure page above; the way you file it is exactly the same.
