What this document is
Immigration uses this to see whether, during your last period of stay, you worked CONTINUOUSLY on proper technical/office work, or had long gaps, or were moved to other duties.
What it must show
- The reporting period (usually from the previous permission until now)
- Each host company + how long you spent at each
- The work done at each place
- Number of days · hours worked
- Any period with NO host company, and why
Where to get it
The dispatch agency draws it up and signs it. There is no Immigration form → one clear A4 sheet is enough.
Notes
A long stretch with no host company is a serious minus — if you have one, explain it head-on in your letter of reasons.
