What this document is
Immigration is tightening oversight of small employers: it wants to know exactly who the representative is — name, date of birth, address, and any criminal or labour/immigration penalties. Missing it makes the file incomplete.
What it must show
Use the Bureau's 参考様式 (link above)
Filled by the company, signed by the representative + company seal
Date it close to the filing date
Where to get it
Your company's HR or general-affairs department. Handing them the form is fastest.
