What this document is

Certificate of NATIONAL tax payment issued by the TAX OFFICE (zeimusho) — the “form 3” (sono-3) version, confirming all 5 national taxes (income tax, etc.) are paid. This is DIFFERENT from the local-tax certificate at city hall.

Notes

Permanent residence scrutinizes tax payment VERY carefully → you need BOTH local (shiyakusho) AND national (zeimusho) tax certificates.
