What this document is
A list of the APPLICANT's relatives in two blocks: 在日親族 (relatives in Japan) and 在外親族 (relatives abroad). The Bureau uses it to grasp the family situation and the relationship with the guarantor.
What it must show
- 6 columns: relationship · name · age · nationality · phone · address
- Scope: spouse, children, parents, siblings (NOT grandparents or cousins)
- Include relatives abroad (city-level address is enough)
- At the bottom: date + the applicant's HANDWRITTEN signature

Download the form from the PDF card above (the Immigration Bureau's form) and print it.
How to get it
- Download + print the form
- Fill in all 6 columns, leave NO blanks (write 不明 if unknown)
- Names must MATCH the passport; content must MATCH your other documents
- Sign by hand + date, submit TOGETHER with the application (not later)

When the whole family applies, EACH applicant still files their own sheet — it is not shared.
